How do I take a policy loan?

How do I file a death claim?

To file a death claim, please have the policy number and contact our service center at 888.513.8797.

How can I take a withdrawal from my life insurance policy?

To withdraw funds from your life insurance policy, please complete the “Request for Disbursement Form for Life Insurance” (ADMIN 5586). To take a loan against your life insurance policy, please complete the "Loan Request Form" (ADMIN 5590). You can mail or fax it to our service center.

Your policy number can be found on page 3 of your policy. You may also contact your agent or our Customer Service Department at 888.513.8797.

How can I obtain a copy of my annuity contract or life insurance policy?

To obtain a copy of your policy, please fill out and submit form “Request a Duplicate Policy” (ADMIN5242) and mail or fax it to our service center.

What is the surrender value of my policy?

What's the current interest rate that I'm receiving?

Please visit our Policyholder Portal or contact our Customer Service Department at 888.513.8797 for questions related to current interest rates. In addition, an annual statement will be mailed to the policy owner on the anniversary date of the policy.

How do I verify the beneficiary on my policy?

The beneficiary information can be located on your original application within your policy, if you have made an update to your beneficiary since the time your policy was issued, please visit our Policyholder Portal or contact our Customer Service Department at 888.513.8797 to verify the current beneficiary on file.

How do I request an in force illustration?

Please fill out the "UL/IL In Force Illustration Request" (ADV1137) form and mail or fax this to our service center. If you have any questions please contact our customer service department at 888.513.8797.

When will I get my annual statement?

Your annual statement is sent automatically on the policy anniversary. The policy date can be located on the information page of your policy.

What is the death benefit amount?

The death benefit amount can be located on the information page of your policy, if you have made an update to your policy since the time it was issued, please visit our Policyholder Portal or contact our customer service department at 888.513.8797 to verify the current death benefit on file.

Can I make online premium payments for my life insurance policy?

At this time we cannot accept premium payments online. You can set up a monthly bank draft and have premiums withdrawn from your checking or savings account via the following methods:

How can I submit my premium payment for my life insurance policy?

If you need to submit a premium payment, you have the following options:

  • You can set up a monthly bank draft and have premiums withdrawn from your checking or savings account via our Policyholder Portal / My Service Requests
  • You can also set up a monthly bank draft and have premiums withdrawn from your checking or savings account. Please complete the "Pre-Authorized Check (PAC) Authorization" (ADMIN 5617) and mail or fax it to our service center
  • If you're paying on a quarterly, semi-annual or annual basis, please mail your payment back to the address indicated on your bill or at the address listed below

Is my policy information available online?

Yes. You can view your policy and submit service requests by registering for our Policyholder Portal.

To log in for the first time, you must have:

  • An active policy number
  • An active email account
  • Social security number or tax identification number (TIN)
If you do not have your policy number, you can contact our Customer Service Department at 888.513.8797. Our customer care representatives are happy to walk you through the registration process.
How do I transfer funds between index options on my annuity contract?

Our annuity products allow you to reallocate funds on your annuity 30 days prior to or up to 25 days after your contract anniversary. A letter and reallocation form will be mailed to you 30 days prior to your anniversary. Should you decide to change your allocations for the upcoming contract year, please complete the form provided and mail or fax it to our service center

How do I update the bank information for my life insurance policy?

To update the bank information we currently have on file for your Life Insurance monthly premium, please complete the "Pre-Authorized Check (PAC) Authorization" (ADMIN 5617) form and mail or fax it to our service center at the address listed below. If you need to stop the automatic bank draft from occurring please contact our customer service department at 888.513.8797. Please note the request must occur 48 hours prior to the scheduled draft.

When is my life insurance premium due?

Your premium is due on the effective date of your policy unless you have requested a specific day of the month. If you need assistance locating this information please contact our Customer Service Department at 888.513.8797.

How do I stop a life insurance premium payment from coming out of my bank account?

If you need to stop the automatic bank draft from occurring please contact our Customer Service Department at 888.513.8797. Please note the request must occur 48 hours prior to the scheduled draft.

How do I cancel my term life insurance policy?

We're sorry to lose you as a customer but understand the need to cancel your coverage. Please draft a letter of instruction to cancel your policy. Include the policy number, cancellation effective date, signature, current date and then mail or fax it to our service center. The cancellation effective date cannot be more than 60 days in the future.

How do I update my beneficiary?

To update the beneficiaries for your Life Insurance policy or Annuity contract, you can log in to our Policyholder Portal and submit an online Service Request or you can fill out the 'Change of Beneficiary Request' (ADMIN 5742). Once you have completed the form, please fax or mail it back to the address below.

How do I convert my term life insurance policy to permanent insurance?

Please contact your agent or our Customer Service Department at 888.513.8797 to review your current conversion options.

How do I change the smoker rating on my life insurance policy?

To submit a request to have the Underwriting Class reconsidered please contact our Customer Service Department at 888.513.8797.

How do I check the status of a service request?

It could take up to 30 days to complete your request depending on the type of form you've submitted. Processing times are indicated in the Download Forms Section under your particular request and do not include mailing times.

If you opened a service request via our Policyholder Portal, the status will be listed under "My Service Requests."
